CarePartners of Connecticut is seeking a compassionate, self-directed Physical Therapist for our Home Health CarePartners team within our Healthcare & Medical Services department. In this role, you will deliver high-quality, evidence-based physical therapy services to patients in their homes across the local Connecticut community, helping them regain mobility, function, and independence while supporting CarePartners’ mission of improving community health through coordinated, patient-centered care. Working closely with physicians, nurses, case managers, and other allied health professionals, you will evaluate patients’ physical function and home environment, develop individualized treatment plans, and provide hands-on therapeutic interventions including therapeutic exercise, gait and balance training, functional mobility training, and patient and caregiver education. You will regularly reassess progress, adjust care plans, and document all visits and outcomes in accordance with regulatory, payer, and CarePartners standards, ensuring clear, timely, and accurate clinical records in our electronic health record systems. The position requires strong clinical judgment, excellent communication skills, and the ability to work both independently in the field and collaboratively as part of an interdisciplinary care team. As a representative of CarePartners in the home, you will build trusting relationships with patients, families, and caregivers, respect cultural and individual differences, and advocate for resources and services that support optimal health outcomes. You will coordinate closely with our provider network to align home health therapy with overall care plans, contributing to reduced hospitalizations, improved recovery, and greater patient satisfaction. Responsibilities include scheduling and prioritizing daily home visits, performing initial and ongoing assessments, establishing measurable goals, implementing safe and effective interventions, instructing patients and families in home exercise programs and fall-prevention strategies, and identifying when changes in condition require escalation to the broader care team. You will adhere to infection control protocols and safety guidelines in the home environment and provide feedback on trends and needs you see in the community, supporting CarePartners’ focus on continuous quality improvement. This full-time role is ideal for a Physical Therapist who is motivated by autonomy in the field, enjoys one-on-one patient interaction in the home setting, and aligns with CarePartners’ culture of collaboration, compassion, and patient-centered care. Candidates should be comfortable using mobile technology for documentation and communication, managing a field-based schedule, and traveling within the assigned Connecticut service area.
