Professional Case Management is seeking a dedicated Physical Therapist to join our Healthcare & Medical Services team, providing in-home therapy services to individuals with complex medical needs, including former energy workers and their families. In this role, you will travel to patients’ homes to complete comprehensive evaluations, develop individualized treatment plans, and deliver evidence-based interventions focused on restoring mobility, reducing pain, and improving overall function and quality of life. You will collaborate closely with nurses, case managers, physicians, and family caregivers to coordinate care, adjust treatment goals, and ensure that each patient receives safe, effective, and compassionate services tailored to their unique conditions and home environment. Day to day, you will perform functional and musculoskeletal assessments, establish measurable therapy goals, and implement therapeutic exercises, manual therapy techniques, gait and balance training, and patient-specific home exercise programs. You will educate patients and families on proper body mechanics, safe use of assistive devices, fall prevention, and strategies to maximize independence at home. Documentation of assessments, progress notes, and care plans will be completed in a timely and accurate manner in accordance with regulatory standards and company policies. You will monitor clinical changes, communicate concerns to the interdisciplinary team, and participate in ongoing care conferences and quality initiatives.
